NOW ACCEPTING NEW PATIENTS 18 years of age and older. We
have expanded our hours to provide service to new patients with minimal
waiting times. Retainer fee does not apply to anyone 18 through
64 years of age unless patient is on Medicare. Patients 65 and older In
July 2012 we started a retainer-based model for all patients 65 and
older. The retainer model enables us to continue to provide
outstanding service to our patients. The advantages of this model
can be seen in more detail by reviewing the retainer agreement in our
office. Our
practice participates with traditional Medicare Part B, not Medicare
Managed Care replacement plans (i.e., Medicare Advantage Plans) or any
commercial insurance plans.
******************NOTICE******************************* Effective
April 1, 2018 there is a new retainer fee for the practice. In
addition, three of the five members of our coverage group have retired
from practicing Internal Medicine. Therefore, only Dr. Holtz and
Dr. Holubka will be providing practice coverage. In addition, Dr.
Holubka will not be admitting patients to the hospital; we will be
using IMA hospitalists and consulting on your in-patient care as

Jacqueline
Holubka was Co-Division chief, Division of Hospital Medicine at Saint
Barnabas Medical Center (SBMC) - Livingston, NJ. She developed
and managed a large Hospitalist practice group at SBMC, Kessler
Institute of Rehabilitation and several sub-acute rehabilitation
Centers. She was selected for UCSF Fellowship for Hospital
Leaders and she developed an Ambulatory Care Program for Internal
Medicine for residents at St. John Hospital and SBMC. Dr. Holubka
returned to private practice and has been working with Dr. Holtz since
2012.
